Good questions!

An unfortunate KingAir pilot collided with a communications tower a few years
ago, while on an SDF approach at Somerset, KY. ATC cleared him to this
approach, though it later turned out the approach had been decommissioned.
(No intended implication here that this is WHY he ended up in the wrong place,
but theres a good chance its related).

Its a wierd approach, and I wouldnt be surprised to learn that the few
remaining are also slated for decommissioning.
